Technology Change Request

Need a change to a District IT system? Start here.

This request is for documenting and getting approval for a planned change to a Technology Services system, such as a software upgrade, a configuration change, a network adjustment, or any work that could affect an IT service. This process follows the District's Change Management Standard, Administrative Procedure 3910.8.

Why this form

A change is the addition, modification, or removal of anything that could affect an IT service. Submitting a change request helps us:

  • Document what is changing, why, and how to back it out
  • Route the change for the right level of review and approval before work begins
  • Schedule changes to avoid conflicts, outages, and restricted periods
  • Keep a clear record of who approved the change and how it turned out

Who should submit a change request

Technology Services and function area staff who are planning a change to a District system. This form is not for reporting something that is already broken (that is an Incident) or for routine requests. 

Choosing the right change type

You will pick one of three change types on the form. If you are unsure, choose Normal and the Change Review Group will help classify it.

  • Standard: A low-risk, routine change that already has an approved procedure (documented as a knowledge base article). Pre-approved through that procedure; you document the outcome in the ticket.
  • Normal: Any planned change that does not have an approved procedure. Pre-approved by the Change Advisory Panel, reviewed by the Change Review Group, then approved and scheduled by the Change Advisory Panel.
  • Emergency: A high-priority change needed to resolve a critical outage or security risk. The Change Advisory Panel is notified and approves, and the outcome is reported at the next Change Review Group meeting.

What to have ready

Have a few basics in mind before you start:

  • A short, clear title for the change (for example, "Upgrade Wi-Fi controller firmware")
  • A brief description of what is changing and why
  • A rollback plan in case the change needs to be undone
  • The service or system affected, and who could be impacted
  • Your proposed start and end dates, avoiding any restricted change windows
  • Any related procedure (linked as a knowledge base article) rather than repeated steps
